No. Provision is a free program for people with Medicaid. There is no cost to find out what services you may be able to get. If you qualify, there is no cost for the services either.

New York State Medicaid offers these services because staying healthy involves more than doctor visits. Things like what you eat, the conditions you live in, and the stress in your life affect your health too. Medicaid funds services that help with these things. Our job at Provision is to check if you qualify and arrange services for you if you do.

No. Provision helps you access the services that are already available to you through Medicaid. You choose whether to take part, and you can stop at any time. Your other Medicaid benefits will not be affected.

Usually, you can get the services, for example free meals or help with your rent, for 6 months at a time. If you still need the services after that, they may be renewed for another 6 months.

Even if you're not eligible, others in your household may be. Check for your whole family. Also, even if no one in your household qualifies right now, you may be able to get support in the future, if something changes. You can call us back if anything changes. For example, if you become pregnant, you may then qualify.

A Provision Advisor will call you within a few working days to learn more about your situation and explain your options.

On the form, you will need to fill out some details about yourself so that we can find out if you qualify for support services. We'll need:

  • your address
  • your Medicaid coverage status
  • your Medicaid CIN number
  • details of family members on Medicaid
  • how you'd like to receive support (for example, would you like to receive cooked meals or do you prefer groceries to cook yourself?)

Provision connects you with services that make day-to-day life a little easier and help you stay healthy. Depending on your situation, you may be able to get help with:

  • Food — home-delivered meals, fresh groceries, cooking supplies, and meals made to fit your health needs.
  • Housing — help with rent, utility bills, and home problems like mold or rodents. Support finding or moving into a new home.
  • Transportation — help getting to and from medical appointments.
  • Guidance — Provision Advisors available to help you find out what you can get and make sure you get it.
